You cannot have a negative number of channels, let's pass it as
uint8_t instead of int8_t.

This updates the copyright header to all be the same, which is
pretty much an update of where to mail request for a copy of the GPL
and the years of the MPD project. This also puts all committers under
'The Music Player Project' umbrella. These entries should go
individually in the AUTHORS file, for consistancy.

Some 24 bit code can be reused. The 32 bit variant has to use 64 bit
integers, because 32 bit integers could overflow. This may be a
performance hit on 32 bit CPUs.

Replace a "static" buffer with the PCM buffer library.

The 24 bit implementation is mostly copy'n'paste of the 16 bit
version, except that the data type is int32_t instead of int16_t.

Separate code from pcm_utils.c to keep it small and simple.
