shout: removed DISABLED_SHOUT_ENCODER_PLUGIN
Having an array with disabled entries sucks. Removed that
DISABLED_SHOUT_ENCODER_PLUGIN macro, and fill the plugin list only
with plugins which are actually enabled. This should be done for all
plugin types.
